Upon completion of the following you will be directed to the appropriate registration form.
PAYMENT for participation in this tournament is due prior to the event. Unpaid balances are subject to a late fee 30 days after the event. By registering for this tournament you understand you are responsible for on-time payment and certify that you are authorized to enter into a contract on behalf of the billing party.
On registration form, under Contact section, fill in the address, city, state and zip code to which you would like the invoice and event details to be mailed.
IMPORTANT: If INVOICE for this tournament is to be sent to a second party, I have inserted the correct names and addresses on the registration form, and will followup to insure payment is made in a timely manner.
Player names must be submitted no later than September 1, 2017. For substitutions after this date, please have player check in under the the original registered name(s).
Cancellation requests must be received via e-mail in the Foundation office no later than September 1, 2017, or you will be responsible for the full amount of the fees.
Upon completion of the online registration process a confirmation email will be sent from the Foundation. Your registration is NOT COMPLETE until this confirmation e-mail is received. If you do not receive confirmation e-mail within two business days, please call the Foundation at 949.837.5291, to secure your spot(s) in the tournament.
